Its high functioning interdisciplinary teams work to fully integrate and coordinate medical and behavioral health care, community resources, and in home support services. This is a rewarding opportunity to help frail and disabled older adults remain in a community setting and thrive.The physical therapist evaluates and treats patients in multiple settings (including the adult day health center and participant's homes) in accordance with CHA policies and procedures, federal and state regulations, professional standards of care, and APTA guidelines and code of ethics,. The physical therapist is a member of the interdisciplinary team and participates in performance improvement projects, collaborates with other members of the rehabilitation department, supervises students, and completes all documentation and administrative duties appropriately. Essential job functions are focused on provision of quality care, service excellence, safety, cultural competence, and performance improvement.Qualifications:Bachelors, Masters, or Doctoral Degree in Physical Therapy requiredMassachusetts LicenseAmerican Heart Association BLSPrevious experience preferredMust have a reliable source of transportation for home visits[cha013017].
